Basketball (Y5&6) Terms 2&3 Registration Form

PAYMENT IS REQUIRED AT THE END OF THIS REGISTRATION, IF YOU DO NOT MAKE PAYMENT THEN YOUR REGISTRATION WILL NOT BE SENT TO ME.

You will receive a confirmation email if your registration is successful. If you do NOT receive an email (please ensure you have checked your junk/spam folders) then I likely havent received your registration, please contact sport@pillanspoint.school.nz

This is really important to get CORRECT as this is how the teams are sorted!

Please make sure the contact email and number you put in this form are CORRECT as this is how all correspondence will reach you from the sports coordinator and your team manager.

Are you happy for your e-mail address and mobile number to be given to Tauranga City Basketball for registration purposes? *
Did your child play Mini Ball or Basketball in Term 2 & 3 last year? *
If yes, please state which team they played in last year.
Are you available to coach a team? *

Coaches hold the team practises and are present to coach during games.
Coaches/Managers will need to complete score sheets at least 10 minutes prior to the start of the game. Teams must sit on the right benches as determined by the score sheets (refer to rules). Both coaches must sign the score sheet at the completion of the game to confirm the result is correct.
There are resources available to help with these roles and they can be shared across parents if required. Even if you haven't done any of them in the past- it's a great opportunity to get involved with your child's sport.
Without parents volunteering for these roles; we are unable to submit teams.

Are you available to Manage a team? *

Managers are responsible for team logistics & organisation e.g. correspondence of practise and game times to all members of the team; key point of contact for PPS Sports & Events Co-ordinator & Tauranga Basketball. Managers are also responsible for collecting the team's uniforms at the end of the last game; cleaning them and returning the set of uniforms to the office 7-10 days after the last game.

Do you have any requests for team placement?

NB: These are requests to do with transport & logistics (e.g. my child needs to be in the same team as XX as we will carpool). OR if your child is in a lower year level but is ready to play UP a team (eg Year 4 who wants to play in Y5&6 Basketball team). Whilst we will do our best to fulfill all requests, this isn't guaranteed. Please email sport@pillanspoint.school.nz if you need to send further information/explanation. Type N/A if you have no requests.
