WHICH FORM SHOULD I FILL IN?
1. Has your child tested POSITIVE?
If yes, please fill in the form below;

If you have completed this form you DO NOT need to let us know via HERO every day that your children are going to be away. We will take the information from this form and use it to mark your child as absent for 7 days and we will look forward to seeing them back on day 8.

2. Are your children at home self-isolating because you have a COVID POSITIVE case in your household?
If yes, please fill in the form below;

If you have completed this form you DO NOT need to let us know via HERO every day that your children are going to be away. We will take the information from this form and use it to mark your child as absent for 7 days and we will look forward to seeing them back on day 8.
If during the self-isolation period one of your children tests positive please complete the 'Positive Student Case Form'.
3. Do your children have COVID symptoms, are they away for another medical reason or are you choosing to keep your children home while there are many COVID cases in our community?
If yes, please fill in the form below;
PPS FORM FOR CHILDREN WHO ARE UNWELL OR WHERE FAMILIES ARE CHOOSING TO KEEP THEIR CHILDREN AT HOME
If you have completed this form you will need to let us know via HERO every day that your child is absent from school unless you are able to give us an accurate timeframe of how long your child will be absent in your initial message. The more information you give us, the less chance we will need to contact you for more information.
If anyone in your household tests positive while you are all still at home please complete one of the above forms to let us know.
HARD COPY LEARNING PACKS AND DEVICES
These are available for;
- COVID positive students
- students who are self-isolating as household contacts
- any other students who are at home
Please fill in one of the online forms above and a hard copy pack will be organised for your children.
If the request is made before 9am it will be ready for pick up that afternoon.
If the request is made between 9am and 3pm it may not be ready for collection until the next day. We are working as hard as we can to get these sorted as soon as we can for you
If you don't have anyone to pick the pack up for you please write this in the comment section and we will be in touch to make a plan for the pack to be delivered.
